Please find below the various consent forms relating to Junior / Young Golfers.
Please complete and submit all four consent forms. The forms will be stored securely by GCGU.
Please submit these forms even if you have signed similar forms previously as these forms have been updated in line with Safeguarding guidance issued by Wales Golf.
The forms need to be signed and submitted before a Junior golfer can take part in the GCGU winter coaching scheme.
